Terms and Conditions

ATTENDANCE: Frontline Health LLC adheres to all national, state, local, and governing agency guidelines, rules, and policies. All classes, courses, and programs require participants to be physically present and actively participating for the alloted scheduled times. Required educational information is delivered, and skills performances are conducted, during these times. Arriving late, leaving early, or missing any portion of a program is disruptive to other participants and distracts from the delivery of necessary and required program content. As such, Participants arriving more than fifteen (15) minutes beyond the scheduled program start time will not be permitted to attend or participate. Additionally, participants must be present during the entirety of the program, until such time as the program instructor officially ends the course. Those who depart early, miss more than fifteen (15) minutes of active course time, or are not present for any mandatory content, will not be eligible to receive a course completion card or certificate and are not eligible for a refund or reschedule. You may call 212-983-5389 or email shane@frontlinehealth.com to reschedule for a future class at no additional charge.  Please contact us right away if you need to reschedule your class.  In the event that you miss your class without calling or emailing in advance to let us know, we will allow you to reschedule once for a future class at no additional cost.  If you fail to attend your rescheduled class, then you will have to re-enroll and pay to attend a future class.

ILLNESS / PANDEMIC: At Frontline Health LLC, the safety of our students and instructors is our utmost concern. We take precautions to minimize the risk of disease transmission during CPR classes to make sure you have a safe training environment. We train according to, and use equipment that meets the standards of the American Red Cross and American Heart Association. Whenever possible we use single use disposable equipment. Some equipment such as CPR manikins and AED training devices are used repeatedly. All equipment is thoroughly sanitized after every single use according to the manufacturers recommendations and CDC guidelines for infection control. Each student in our training center will have their own sanitized CPR Manikin, CPR Pocket mask, and disposable single use one way valve. You will NOT be asked to put your mouth directly on a Manikin. Classroom surfaces, door knobs, and other touch points are cleaned after every training session. All participants and instructors are encouraged to practice good hygiene, including hand washing with soap and water for at least 20 seconds before and after the course, and before and after snack or meal breaks. We encourage students to stay home when you are sick and avoid close contact with people who are sick. If a scheduled student has identified symptoms (fever, cough, shortness of breath), they are advised to not attend a training class. You will be able to postpone your class for any reason and will NOT have to pay to reschedule. We encourage you to avoid touching your eyes, nose, and mouth. We encourage you to cover a cough or sneeze with a tissue, then throw the tissue in the trash. Our classrooms are stocked with alcohol-based hand sanitizer and cleaning wipes for your use during CPR class.

GUESTS: Registered participants may not bring additional non-registered guests or observers such as, but not limited to, friends, family, co-workers, children, spouses, etc, into the classroom. Anyone who is not an officially registered participant may not be present during any portion of class, as this may be disruptive to you, other participants, and / or the Instructor(s). There is a small waiting area in the lobby for guests. Those who require an interpreter to be present must indicate such at the time of enrollment. Frontline Health LLC does not offer interpretive services, but does permit an interpreter to attend with a registered participant. Accompanying interpreters are not eligible for a course completion card or certificate unless they are also a formally registered participant and complete all course requirements.

PETS: Animals and pets are prohibited from being present during any part of a course, class, event, or training. Participants, whether registered or non-registered, may not bring pets or animals of any kind. Service animals are permitted, but must be clean, non-disruptive, tame, non-violent, and trained to use the bathroom outdoors. Those bringing a service animal are responsible for the service animal's actions and for cleaning up after them.

AUTHORITY TO REGISTER AND/OR ACT AS AGENT: You represent and warrant to Frontline Health LLC that you have full legal authority to complete this class registration on behalf of yourself and/or any party you are registering (the "Registered Parties"), including full authority to make use of the credit or debit card to which registration fees will be charged. If you are registering a child under the age of 18 or an incapacitated adult you represent and warrant that you are the parent or legal guardian of that party and have the legal authority to enter into this agreement on their behalf and by proceeding with this event registration, you agree that the terms of this Agreement and Waiver shall apply equally to all Registered Parties.

COMMUNICATION: Customers are responsible for reading all emails. Frontline Health LLC. is not responsible for any emails misplaced in Junk, Spam, or Promotions folders. However, we will be more than happy to resend any information needed.

CURRICULUM:

American Red Cross Licensing and Certification Standards: Frontline Health is a licensed training provider for the American Red Cross (“ARC”) and is required to deliver instruction, evaluate participants, and issue certifications strictly in accordance with ARC program standards and life-saving training objectives. These standards establish specific required skills, assessments, and performance criteria that must be successfully completed in order to earn certification. Registration is open to all individuals. However, certification is not guaranteed and is contingent upon the participant’s ability to successfully complete all ARC mandated objectives, including required physical skills and evaluations. Participants who do not meet these objectives will be marked as Unsuccessful and will not receive American Red Cross certification.

American Heart Association Licensing and Certification Standards: Frontline Health is an American Heart Association training site (“AHA”) and is required to deliver instruction, evaluate participants, and issue certifications strictly in accordance with AHA program standards and life-saving training objectives. These standards establish specific required skills, assessments, and performance criteria that must be successfully completed in order to earn certification. Registration is open to all individuals. However, certification is not guaranteed and is contingent upon the participant’s ability to successfully complete all AHA mandated objectives, including required physical skills and evaluations. Participants who do not meet these objectives will be marked as Unsuccessful and will not receive American Heart Association certification.

Health and Safety Institute Licensing and Certification Standards: Frontline Health is a Health and Safety Institute Training Center (“HSI”) and is required to deliver instruction, evaluate participants, and issue certifications strictly in accordance with HSI program standards and life-saving training objectives. These standards establish specific required skills, assessments, and performance criteria that must be successfully completed in order to earn certification. Registration is open to all individuals. However, certification is not guaranteed and is contingent upon the participant’s ability to successfully complete all AHA mandated objectives, including required physical skills and evaluations. Participants who do not meet these objectives will be marked as Unsuccessful and will not receive HSI certification.

WAIVER: You understand that participation in any class, course, or program is potentially hazardous, and that a registered party should not participate unless they are medically able. You understand that there are risks and dangers inherent when participating in emergency care training classes. These risks include, but are not limited to, injuries during skills practices or acquiring communicable diseases. You understand that it is the responsibility of you and/or the Registered Party to inform Frontline Health LLC or its representatives if they have a medical condition that would limit their participation in a class and/or their completion of required skills. You understand that Registered Parties should not practice moving simulated victims if they have a history of back or knee problems as such practice may aggravate previous injuries. You understand and agree that in consideration of being permitted to participate in the class, you and/or any Registered Party, their heirs, personal representatives or assigns of you or the Registered Party do hereby release, waive, discharge and covenant not to sue Frontline Health LLC for any and all liability from any and all claims arising from participation in the event by you or any registered party.

INDEMNIFICATION: You hereby voluntarily release and hold Frontline Health LLC, and each of their officers, agents, employees, volunteers, and contractors, harmless from any and all liability or costs from injury associated with or arising from you and Registered Parties participation in any class or event held by Frontline Health LLC, including from negligence. You and Registered Parties understand and agree that this Release applies to personal injury, including death, and property damage which you and Registered Parties may suffer. You and Registered Parties understand and agree that this Release will be binding on you, Registered Parties, as well as you and Registered Parties spouses, heirs, personal representatives, assigns, children, and any guardian appointed by the court for said children.

BY INDICATING YOUR ACCEPTANCE OF THIS AGREEMENT AND WAIVER, YOU ARE AFFIRMING THAT YOU HAVE READ AND UNDERSTAND THIS AGREEMENT AND WAIVER AND FULLY UNDERSTAND ITS TERMS. YOU UNDERSTAND THAT YOU ARE GIVING UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE. YOU ACKNOWLEDGE THAT YOU ARE SIGNING THE AGREEMENT AND WAIVER FREELY AND VOLUNTARILY, AND INTEND BY YOUR ACCEPTANCE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
